Is there any reason a 000 guitar has to have a slotted headboard? And a 13/16" wide at the nut neck?

I've got small hands and would like to keep the neck as narrow as possible without compromising sound quality and playability.

Put on any kind of neck made to the right scale length that you want. Did you mean 1 13/16". Martin sells 000s with 1 11/16" SO you can go 1/8" thinner and still have a factory size. 1 3/4" is sold as well.

Ideally you should size the neck to what's best for you. A triple 0 can be a paddle head or a slot head -- and spokeshave/sandpaper can bring your personal preference into an ideally sized & shaped neck.
Traditionally a 25.4 12 fret neck on a 000 has been a slot head and a 24.9 and 25.4 14 fret 000 have been paddle heads. Rules can be broken.

My first guitar, theoretically a 14-fret OM, has a 1-13//16" wide neck and a headstock with blind slots (they don't go all the way through) because I like that look.,

000's can vary indeed from 1 11/16 to 1 7/8 nuts
much depends on the time period.
as for what your building I agree with what others are saying , do it the way you want. I will add one thing what plan are you using. Many plans out there and many are not very good.

The 000 can be 14 fret or 12 fret it can also be long or short scale. Martin changed the shape of these many times since the inception . I am a Martin repair center and will be happy to point you in the right direction as will many here.

I made 12 fret triple O with John's help. It has a 1-11/16" nut as that is what I prefer. I made it 4-1/2" thick (mistake because you can't buy a standard case for it!). Even though it is mahogany, I can't tell its sound from my 1973 000-28 in rosewood - both are fantastic and about the same.
