It looks as though the notches may help the thread act as a tap as it's going in and make them easier to install. Where do you get those?

The self-tapping (slot goes in the wood) insert John uses is the way to go IMHO. My experience is limited to setting five and from that I can suggest that the screwdriver approach is not the way to go. Because I didn't have a standing drill press for the first two, I used the screwdriver method -- end result was undesirable. The next two I used the purpose-made T-wrench and the result was okay, but not really satisfactory. The last one I used the drill press to drill the hole and also to insert turning the chuck holding the shoulder bolt by hand -- result was an insert in proper alignment.

I tried a few inserts from Steel Zinc and these brass ones. In all the split inserts I hated the chance to catch and cross thread. Martin showed me how then used them and I put them in slot down ever since.

I have nothing but trouble trying to thread these things into mahogany end grain, that is why I'm switching to the barrel nuts and bolts

I have had no problem
what size drill bit? I have a big issue as an engineer with barrel bolt in that you have less wood holding unless you set them deeper. Can you post pictures of what is happening? Maybe we can help you,

In hard wood, you may definitely need to drill the holes larger than what is specified for a given insert.

I think I use a 5/16 or 11/32
I like to go 1 size up and I will use tite bond just to fill

I probably do have a pic, I'll look. The end grain has a tendency to crumble, however, I may be using the incorrect insert. The ones I've used have a finer thread on them.
